ADOPTION PLAN FOR ANNUAL HEARING CARE
Cost is $752 per year
Designed for those who are fit with hearing devices elsewhere but need a trusted, long-term partner to take over their hearing care; devices must be less than 5 years old. With the Adoption Plan, you gain the team at Sound Relief to provide expert service, advice, and device maintenance to maximize your investment in better hearing.
This 12 month service plan includes:
Three visits with your Audiologist
At the initial appointment, we will uncover any issues that could be resolved to improve the hearing experience in this comprehensive appointment that includes:
Detailed case history to understand any issues
Otoscopic exam to verify ear canal health
Wax removal, if needed
Comprehensive hearing test to update prescription
Professional deep cleaning of devices
Real Ear Measurements to verify that the new prescription is met
Fine-tuning adjustments as needed
Smartphone pairing and/or app orientation when appropriate
Two additional follow-up appointments with an Audiologist
When: 3 - 6 Months and 6 – 12 Months
What is covered: Programming and performance adjustments, clean devices, check for hardware/software updates, review data logging, check for earwax and remove if needed
Ongoing Support with an Audiology Assistant (as needed)
Clean & Checks
Supplies provided for your devices (i.e., wax filters and domes – 4 per year max)
On-site lab repairs when possible (service and labor)
If your devices ever need repair during the manufacturer’s warranty, we’ll take care of everything for you, including sending them in at no cost.
